Questionnaire Machine

The Questionnaire Machine is a PHP script aimed at creating and administering questionnaires online. The questionnaires, as well as a set of logins and passwords for the participants, can be created very flexibly. Upon completion of a questionnaire, an e-mail is sent to the administrator with the results.
